Work Environment

The work setting for this role is primarily onsite at our assisted living facility in New Hyde Park. You will be part of a dynamic and collaborative healthcare team, committed to providing exceptional care in a supportive and patient-friendly environment. Our work hours typically span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics scheduled until 8:00 p.m., as well as community outreach events on weekends. We aim to maintain an average of about 40 hours per week for this position.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, using evidence-based practices.
• Diagnose and manage both acute and chronic medical conditions, incorporating preventive care and health promotion strategies.
• Develop and implement health education programs in collaboration with community organizations to address prevalent health issues.
• Provide culturally sensitive care, advocating for patient rights and ensuring equitable access to necessary healthcare services.
• Coordinate patient referrals and follow-up care, ensuring a seamless continuum of care for our residents.
• Maintain accurate and comprehensive medical records, adhering to regulatory standards and ensuring patient confidentiality.
• Participate actively in community outreach events and health fairs to promote health awareness and facilitate health screenings.
• Stay informed about community health trends, emerging evidence-based practices, and pertinent healthcare policies.

Patient Population / Client Focus

Your primary focus will be on the elderly and diverse individuals residing within our assisted living facility and the surrounding New Hyde Park community. You will work closely with residents who often have multifaceted health needs due to age, chronic conditions, and various social determinants of health. Your role is crucial in advocating for their health, autonomy, and overall well-being.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license.
• A Master’s degree in Nursing from an accredited institution.
• A minimum of 3 years of clinical experience, particularly in community health or public health settings.
• Strong interpersonal and communication skills, with an ability to effectively engage and work with diverse populations.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of USD 180,000 - 200,000 per year, commensurate with experience and qualifications.
