Work Environment

You will be working onsite at our hospital facility, providing direct patient care in a fast-paced, collaborative setting. The role requires an average workweek of approximately 40 hours, with operating hours from 9:00 a.m. to 6:00 p.m., and a possibility of evening clinics until 8:00 p.m. Your involvement in community outreach events and health fairs may include weekends, further emphasizing our commitment to service.

Key Responsibilities

• Perform thorough health assessments and screenings for patients of all ages, ensuring comprehensive evaluations are conducted.
• Diagnose and manage diverse acute and chronic medical conditions, focusing not only on treatment but also on preventive care and health promotion strategies.
• Collaborate with community organizations to design and implement impactful health education programs tailored to address specific community needs.
• Provide culturally sensitive and empathetic care while advocating for patients’ rights and improved access to healthcare resources.
• Coordinate referrals and ensure comprehensive follow-up care to maintain continuity in patients’ treatment plans.
• Accurately maintain detailed medical records and documentation in accordance with regulatory standards.
• Engage actively in community outreach initiatives and health fairs, contributing to public health awareness and delivering necessary screenings.

Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in New York State.

Compensation and Benefits

The position offers a competitive salary range of USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
