Work Environment

Key Responsibilities

• Conduct comprehensive patient assessments, including obtaining detailed health histories and performing thorough physical examinations tailored to cardiovascular conditions.
• Utilize advanced diagnostic skills to order, interpret, and analyze test results, leading to informed diagnoses and management of both chronic and acute cardiac issues.
• Formulate and implement individualized patient care plans that encompass health maintenance, chronic disease management, and preventive strategies, employing established clinical protocols.
• Monitor and evaluate ongoing patient progress, collaborating with supervising physicians to adjust treatment plans as necessary and communicate updates to the patients and their families.
• Provide education and counseling to patients regarding their conditions, treatment plans, and lifestyle modifications, empowering them in their health journey.
• Coordinate efforts for hospital admissions and communicate effectively with inpatient teams to facilitate seamless patient transitions and safe discharges.
• Collaborate with multidisciplinary specialists, including social workers and nutritionists, to develop comprehensive care strategies that address all aspects of patient wellness.
• Responsible for maintaining accurate and timely documentation of patient encounters within the electronic medical records (EMR) system.
• Engage in research protocols aimed at enhancing clinical practices and ensuring evidence-based care methods are applied consistently.
• Maintain a safe and therapeutic work environment for patients and staff, adhering to best practices in healthcare delivery.

Patient Population / Client Focus

This role involves working primarily with patients with cardiovascular conditions, including adults facing chronic heart failure, pulmonary arterial hypertension, and post-transplant care. The NP will be instrumental in providing care to patients undergoing complex treatments and therapies, including outpatient intravenous medication support.

Qualifications

• A Master’s Degree in Nursing from an accredited program, with certification as a Nurse Practitioner specializing in adult care.
• Possession of current New York State Registered Professional Nurse and Nurse Practitioner licenses, along with NP board certification.
• Active certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), as well as a valid DEA and NPI registration.
• Proficient in various computerized systems, including electronic medical records and standard office software applications like MS Word and Excel.

Preferred Experience

• Minimum of two years of clinical experience in a hospital or outpatient setting, preferably with a focus on cardiology.
• Experience managing patients with advanced heart failure and those considered for innovative therapies.
• A strong background in patient education and coaching, especially for those navigating chronic illnesses.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with experience and qualifications.
