Position Overview

Gundersen Health and Wellness Clinic is currently seeking a dedicated and experienced Nurse Practitioner (NP) to join our team at our Assisted Living Facility in Tarrytown, New York. In this full-time, onsite role, you will be instrumental in delivering high-quality healthcare services under the auspices of the Center for Advanced Cardiac Care (CACC). You will work closely with the Chief Nurse Practitioner and the CACC Medical Director to ensure a comprehensive, patient-centered approach in our outpatient cardiology practice.

Work Environment

Our assisted living facility is designed to promote independence while ensuring patients receive the necessary medical attention and support. As an NP within this facility, you will collaborate with an array of healthcare professionals, including physicians, registered nurses, social workers, and pharmacists, within a therapeutic and supportive setting. The facility is equipped with modern medical technology, electronic medical record systems, and resources to enhance patient care and ensure seamless communication among the care team.

Key Responsibilities

As a Nurse Practitioner in our Advanced Cardiac Care clinic, your day-to-day responsibilities will include:

• Conducting thorough patient assessments, including obtaining detailed health histories, performing comprehensive physical examinations, and identifying health discrepancies.
• Ordering and interpreting a variety of diagnostic tests, diagnosing, and managing both chronic and acute cardiovascular conditions.
• Formulating personalized care plans that address health maintenance, provide anticipatory guidance, and outline necessary nursing or medical interventions.
• Collaborating actively with the multidisciplinary team to create and adjust treatment plans based on patient progress and emerging needs.
• Delivering education and counseling to patients and their families on managing cardiovascular diseases and promoting healthy lifestyles.
• Coordinating with inpatient teams for smooth transitions of care, facilitating safe patient admissions and discharges.
• Documenting all patient interactions and care plans in our electronic medical records system, ensuring compliance with documentation standards.
• Participating in research protocols aimed at improving patient care practices in cardiology.
• Contributing to a safe and therapeutic environment for residents and their families by adhering to established guidelines and placing priority on patient safety.
• Engaging in continuous quality improvement initiatives and training to enhance professional development and clinical skills.

Patient Population / Client Focus

You will primarily focus on adult patients with complex cardiovascular conditions, including those with advanced heart failure, pulmonary arterial hypertension, and post-heart transplant care. Your expertise will guide you in developing treatment regimens for patients undergoing advanced therapies and those requiring outpatient support for intravenous or inhaled medications. By providing exemplary care tailored to a diverse patient population, you will directly influence their health outcomes and overall well-being.

Qualifications

The ideal candidate for this position will possess the following qualifications:

• A Master’s degree in Nursing, with specialized training as a Nurse Practitioner in adult care.
• Valid New York State Registered Professional Nurse and Nurse Practitioner licenses.
• NP Board Certification to ensure compliance with professional standards.
• Certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in using electronic medical record systems and other computer-based applications.

Preferred Experience

While a minimum of two years of clinical experience in a hospital or outpatient setting is required, candidates with specific experience in cardiology, particularly with advanced therapies and management of complex heart conditions, will be highly regarded. Familiarity with the intricacies of patient care related to advanced heart failure and post-transplant patient populations is especially advantageous.

Compensation and Benefits

Our comprehensive compensation package offers a competitive salary range of $120,000 – $220,000 per year, commensurate with experience and credentials. Additionally, we provide:
