Key Responsibilities

• Conduct thorough health assessments and perform comprehensive physical examinations on patients, identifying health needs and risk factors.
• Order, perform, and interpret diagnostic tests such as blood work and X-rays to guide clinical decisions and treatment plans.
• Develop and maintain collaborative care plans that are patient-centered, ensuring involvement of the patient and family in the decision-making process.
• Prescribe medications and implement treatment strategies that align with best practices in family medicine.
• Provide tailored health education and counseling tailored to the families about preventive care, nutrition, and managing chronic conditions.
• Participate actively in team meetings and interdisciplinary case conferences to discuss patient care strategies and progress.
• Document patient encounters in accordance with organizational policies, ensuring the maintenance of accurate and thorough medical records.
• Coordinate with a variety of healthcare professionals to ensure holistic care is provided, encompassing physical, mental, and emotional health.

Patient Population / Client Focus

Your primary focus will be on a diverse range of patients, predominantly children and adolescents attending the school. You will address their health concerns, monitor their developmental milestones, and provide preventive care. A particular emphasis will be placed on establishing a trusting relationship with both children and their families to facilitate smooth communication and better health outcomes. Qualifications

• Must possess a valid and unrestricted Nurse Practitioner license in New York State, ensuring adherence to all legal and professional standards.
• A minimum of 2 years of clinical experience as a Family Nurse Practitioner, demonstrating advanced clinical judgment and problem-solving skills.
• Comprehensive knowledge of state and federal healthcare policies, procedures, and protocols.
