Work Environment

This role is based entirely onsite, offering a unique work environment where you will conduct house calls and provide in-person assessments. Key Responsibilities

• Conduct thorough physical examinations and comprehensive health assessments to evaluate the well-being of patients.
• Diagnose and manage acute and chronic health conditions, tailoring each treatment plan to meet the individual needs of the patient.
• Prescribe medications, monitor patient responses, and adjust treatment plans as necessary to ensure optimal health outcomes.
• Administer immunizations and vaccinations to patients of various ages, adhering to current health guidelines.
• Provide essential counseling on health promotion strategies, disease prevention techniques, and lifestyle adjustments that will benefit the patient’s overall health.
• Order, conduct, and interpret diagnostic tests, ensuring timely delivery of results to enhance patient care.
• Collaborate closely with a network of healthcare professionals, coordinating comprehensive care across various disciplines and specialties.
• Maintain accurate, detailed, and up-to-date medical records in accordance with organizational policies and regulatory requirements.

Patient Population / Client Focus

In this role, you will serve a varied patient population, including infants, children, adults, and seniors. Our concierge medicine model allows you to spend more time with each patient, fostering strong relationships and a deeper understanding of their health needs. Qualifications

• A Master’s degree in Nursing from an accredited program.
• An active Nurse Practitioner license in the state of New York, with current certification as a Family Nurse Practitioner.
• A minimum of 2 years of experience working directly with patients across all life stages, demonstrating a strong foundation in family medicine.

Preferred Experience

• Experience in concierge medicine or similar primary care settings where personalized patient care is emphasized.
• Proficiency in electronic health record (EHR) systems to enhance productivity and improve patient outcomes.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
