Work Environment

You will work onsite at our outpatient office set in a welcoming and modern facility designed for patient comfort and efficient care delivery. The clinic operates Monday through Saturday, with specific hours running from 9:00 a.m. to 6:00 p.m.; occasional evening clinics may extend hours to 8:00 p.m. We encourage participation in community outreach initiatives and health fairs, occasionally scheduled on weekends, to engage actively with the community.

Key Responsibilities

• Perform comprehensive health assessments for patients, ensuring a thorough understanding of their medical histories and current health statuses.
• Diagnose and manage a variety of acute and chronic health conditions, creating evidence-based treatment plans tailored to individual patient needs.
• Lead and coordinate community health education programs, partnering with local organizations to disseminate vital health information that promotes preventative care practices.
• Provide culturally competent care, fostering an inclusive environment, and advocating for the rights of patients from diverse backgrounds.
• Ensure continuity of care by coordinating referrals to specialists and managing follow-up processes effectively.
• Maintain accurate and detailed medical records in compliance with legal and organizational standards, facilitating seamless team communication concerning patient care.
• Actively participate in community health events, offering screenings and educational resources to increase health awareness.
• Remain updated on current trends in community health, practices, and relevant healthcare policies to enhance your professional skill set.

Patient Population / Client Focus

Your role will focus on diverse patients ranging from children to seniors, with particular attention to underserved populations in the community. By supporting families with various health needs, from routine check-ups to chronic disease management, you will play an integral part in enhancing community health and nurturing long-term patient relationships.

Qualifications

• Current Board Certification as a Family Nurse Practitioner.
• Valid and active nursing license in the state of New York.
• Bachelor's degree in nursing; a Master's degree from an accredited NP program is highly preferred.
• At least 3-5 years of clinical experience, particularly in community health or public health settings.

Compensation and Benefits
