Work Environment

This position is based onsite, allowing for direct interaction with patients and families in a welcoming home care facility. You will work alongside a multidisciplinary team of healthcare professionals in a supportive and dynamic environment. Our typical working hours are Monday to Friday from 9 AM to 5 PM, providing a balanced work-life integration while serving your community effectively.

Key Responsibilities

• Conduct thorough health assessments for individuals and families, gathering comprehensive medical histories and performing physical examinations tailored to community health needs.
• Diagnose, treat, and manage a variety of acute and chronic medical conditions, implementing preventive care strategies and health promotion interventions.
• Create and maintain accurate health records using electronic health record (EHR) systems, ensuring compliance with healthcare regulations and facilitating seamless care coordination.
• Establish partnerships with community organizations to develop and roll out informative health education programs aimed at increasing health literacy.
• Advocate for patients' rights and accessibility to healthcare services, providing culturally sensitive care that respects diverse backgrounds and experiences.
• Coordinate referrals to specialists and ensure follow-up care, fostering continuity of treatment for patients.
• Actively participate in community events and health fairs, promoting health awareness and providing screenings.
• Stay informed on the latest community health trends, evidence-based practices, and relevant policies that affect patient care.

Patient Population / Client Focus

You will primarily serve diverse populations in Orangeburg, NY, including underrepresented communities often facing barriers to healthcare access. Your role will focus on improving health outcomes through tailored care plans that consider each patient’s unique circumstances and needs. Your ability to communicate effectively and empathetically with different demographic groups will be crucial for engagement and trust-building.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Active and unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized certifying bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current DEA and NPI numbers required.
• Exceptional clinical assessment, diagnostic, and critical thinking skills.

• A minimum of 2-5 years of clinical experience in primary care, family medicine, community health, or public health settings.
