Work Environment

This position is based in a supportive hospital environment in Spring Valley, where interdisciplinary collaboration is encouraged. You will work from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Community outreach activities are an integral part of this role, thus weekends may involve participation in health fairs as part of our commitment to community engagement. Expect an average workweek of approximately 40 hours, allowing you to maintain a healthy work-life balance.

Key Responsibilities

• Conduct thorough health assessments and screenings for varied patients, focusing on both individual and familial health needs.
• Diagnose and manage acute and chronic medical conditions, providing preventive care and promoting health education.
• Collaborate with local organizations and stakeholders to develop effective health education initiatives that are tailored to the community's unique needs.
• Provide care that is sensitive to cultural differences, advocating for patient rights and access to health services.
• Coordinate patient referrals and ensure comprehensive follow-up care, demonstrating a commitment to continuity of care.
• Maintain accurate and detailed medical records, abiding by all relevant regulatory standards and ensuring patient confidentiality.
• Involve yourself in community outreach activities, increasing awareness and accessibility of health services through participation in local fairs and events.
• Stay current with trends in community health, evidence-based practice guidelines, and healthcare policies, enhancing both your practice and community service.

Qualifications

• A Master’s or Doctorate degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid and unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a current DEA# and NPI#.
• Demonstrated strong clinical assessment and diagnostic abilities.
• Excellent communication and interpersonal skills that foster teamwork and community trust.
• Proficiency with electronic health record (EHR) systems.

Preferred Experience

• At least 3-5 years of clinical experience in primary care or family medicine, particularly in community health or public health settings.
• Experience with diverse populations, highlighting the ability to provide culturally competent care.

Compensation and Benefits

This role offers a competitive annual salary range of USD 150,000 – 160,000, commensurate with experience. Additionally, we are pleased to offer a sign-on bonus of up to $10,000.
