SLI Rehab & Physical Therapy is seeking an empathetic and dedicated Nurse Practitioner to join our team in Spring Valley, New York. As a vital member of our healthcare program, you will practice within a concierge medicine approach, providing personalized and accessible healthcare services through house calls and onsite consultations.

Work Environment

This role is based in a dynamic clinical setting where you will primarily provide services through on-site visits, as well as house calls. You will work closely with a multidisciplinary team to offer integrated care, assess patient needs, and advocate for community health initiatives. Your typical work schedule will be Monday through Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. and participation in community health events or health fairs on weekends. You will average around 40 hours of work each week.

Key Responsibilities

• Perform comprehensive health assessments and screenings for diverse individuals and families, ensuring every patient receives thorough evaluations tailored to their unique needs.
• Diagnose and manage a range of acute and chronic medical conditions while emphasizing preventive care and ongoing health promotion strategies.
• Collaborate directly with community organizations and stakeholders to create and implement targeted health education programs that educate the public on essential health topics.
• Advocate for the rights of patients and ensure culturally sensitive care is provided, addressing the diverse needs of the populations we serve.
• Facilitate referrals and coordinate follow-up care to enhance continuity and quality of patient care, bridging healthcare services effectively.
• Maintain detailed, accurate medical records in compliance with state and federal regulations, ensuring all documentation reflects the highest standards of quality and care.
• Engage with the community by participating in outreach events and health fairs, conducting health screenings to promote overall wellness.
• Stay informed about the latest community health trends, evidence-based practices, and relevant healthcare policies to continuously improve patient care methodologies.

Qualifications

• A current and valid Nurse Practitioner certification is required.

Compensation and Benefits

SLI Rehab & Physical Therapy offers a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and qualifications.
