Work Environment

This position is primarily onsite at a school facility in Spring Valley, a vibrant community known for its rich cultural diversity. You will work in a nurturing and collaborative atmosphere where you can connect with patients, their families, and school staff. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evenings until 8:00 p.m. and participation in community outreach on weekends. This flexibility allows you to engage with the community beyond clinical settings, fostering stronger relationships and understanding.

Key Responsibilities

• Conduct thorough and comprehensive health assessments and screenings for students and families, identifying both acute and chronic health issues.
• Diagnose and manage various medical conditions, offering not only treatment but also preventive care and health promotion initiatives tailored to community needs.
• Collaborate with local health organizations and stakeholders to create and implement effective health education programs that promote wellness and disease prevention.
• Provide culturally sensitive healthcare, understanding the unique challenges that diverse populations may face regarding access and care.
• Coordinate patient referrals and follow-up care, ensuring all individuals receive continuity of care and support throughout their healthcare journey.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license in New York.

Compensation and Benefits
