This position is located onsite in a home care facility that focuses on delivering exceptional medical services to patients in the comfort of their homes. Here, our Family Nurse Practitioners work collaboratively with a multidisciplinary team of healthcare professionals, including physicians, nurses, and social workers. Key Responsibilities

• Conduct thorough physical examinations and comprehensive health assessments for patients across all age groups.
• Diagnose and treat a wide range of acute and chronic illnesses, providing personalized treatment plans that cater to individual patient needs.
• Prescribe necessary medications and develop holistic treatment protocols to ensure optimal patient outcomes.
• Administer immunizations and vaccinations in accordance with healthcare guidelines to promote preventative health.
• Provide expert counseling and education to patients and families on health promotion, disease prevention, and lifestyle modifications.
• Order, interpret, and follow up on diagnostic tests, ensuring timely and accurate diagnosis management.
• Collaborate with various healthcare professionals to create a cohesive care plan for each patient, enhancing their overall treatment experience.
• Maintain meticulous and up-to-date electronic medical records (EMR), ensuring compliance with confidentiality standards and healthcare compliance regulations.

Qualifications

• A Master's degree in Nursing from an accredited nursing program.
• Possession of an active Nurse Practitioner license in the State of New York.
• Certification as a Family Nurse Practitioner (FNP), validated through nationally recognized boards.
• A minimum of two years of clinical experience as a Nurse Practitioner, with a proven record of working with patients across various age demographics.

Preferred Experience

• Experience in family medicine or primary care settings is preferred but not mandatory.

• Familiarity with telehealth platforms to accommodate patient needs remotely when necessary.

Compensation and Benefits
