Work Environment

The successful candidate will operate onsite in Spring Valley, providing care directly in patient homes. This unique setting allows for a personalized approach to healthcare, ensuring that our Nurse Practitioners can engage directly with patients in a familiar and comfortable environment. You will be supported by a collaborative team of healthcare professionals, including nurses, social workers, and administrative staff, all working together to meet the comprehensive needs of our patients.

Key Responsibilities

• Deliver consistent, high-quality medical care adhering to established national and community standards.
• Collaborate with office staff to enhance and implement efficient office practices, ensuring streamlined operations.
• Serve as a liaison between practice management and hospital administration, addressing both parties' needs effectively.
• Promote a unified identity for F. F. Thompson Health while ensuring compliance with regulatory standards.
• Provide patient management, including assessments, diagnosis, and treatment, while aligning practice and health system goals.
• Safeguard patient interests and advocate for their well-being in all aspects of care.
• Maintain accurate and timely records of patient care, ensuring documentation is completed within two business days post-service.
• Actively participate in team meetings and strategic planning sessions to contribute to the advancement of the organization’s goals.
• Regularly engage with patients and families to gather feedback and adjust care practices as needed.

Patient Population / Client Focus

Our Nurse Practitioner will provide care to a diverse patient population, including adults with chronic conditions, acute health issues, and those requiring preventive health counseling. Your role will entail not only diagnosing and treating medical conditions but also addressing psychosocial factors affecting health outcomes. Qualifications

• Must hold New York State certification/registration as a Family, Adult, or Pediatric Nurse Practitioner.
• AANP Board Certification is required.
• A valid DEA license is necessary for prescribing medications.
• Proficiency in ICD10 coding is preferred to streamline billing and documentation processes.
• Graduation from an AMA-approved Nurse Practitioner program is essential.

Preferred Experience

• Prior experience as a Nurse Practitioner in an outpatient or home care setting is preferred, enhancing your ability to adapt quickly and effectively.
