Work Environment

This position will be performed onsite, primarily in patients’ homes or non-traditional settings such as community centers where marginalized individuals congregate. Key Responsibilities

• Conduct comprehensive health screening services to assess patient wellness and needs.
• Design and implement continuous medical services with a focus on patients living with HIV/AIDS, ensuring a continuum of care that addresses both physical and emotional needs.
• Provide expert advice and counseling to clients regarding their physical and mental health, facilitating informed decision-making.
• Deliver consultation and training to on-site staff and volunteers to strengthen the care network and enhance service delivery.
• Assist with identifying mental health and substance abuse issues, providing counseling and care coordination in collaboration with other healthcare professionals.
• Engage with clients proactively, demonstrating concern for their health, emotional well-being, and socio-economic challenges.
• Complete thorough health histories, perform detailed physical examinations and screenings, and administer on-site testing for HIV.
• Evaluate and manage common, acute, and chronic illnesses, ensuring adherence to approved treatment protocols. Make referrals to appropriate facilities when necessary and conduct follow-up to ensure continuity of care.

Patient Population / Client Focus

Your primary focus will be on an underserved patient population, including individuals facing health disparities often linked to socio-economic factors. This includes clients struggling with chronic illnesses, mental health disorders, and those requiring specialized care for HIV/AIDS. Qualifications

• A valid New York State Nurse Practitioner license is required.
• A Bachelor’s Degree in nursing or a related field.

Preferred Experience

Compensation and Benefits
