Key Responsibilities

• Conduct thorough patient assessments to diagnose and manage both chronic and acute conditions.
• Develop personalized treatment plans in collaboration with residents and family members to address their specific health needs.
• Order and interpret diagnostic tests, adjusting treatment plans based on findings to optimize patient care.
• Maintain accurate medical records using our electronic medical records (EMR) system, ensuring compliance with all regulatory standards.
• Participate actively in care coordination, including family conferences and advanced care planning discussions.
• Engage with pharmacists to address medication management and recommendations for residents.
• Maintain comprehensive documentation and participate in the evaluation of healthcare delivery outcomes.
• Be present at the facility during work hours to guarantee accessible, face-to-face healthcare coverage for residents.
• Support strategic initiatives aimed at improving value-based care throughout the organization.
• Utilize telemedicine solutions where appropriate to connect residents with specialized care.

This role is centered around caring for an elderly population residing in assisted living facilities. Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.

• Active, unrestricted NP state license and DEA license to prescribe medications.

Compensation and Benefits
