Key Responsibilities

• Maintain accurate and compliant documentation within our electronic medical record (EMR) software to ensure effective patient management.
• Collaborate closely with local and regional operations teams and clinical leadership to meet organizational goals and improve healthcare outcomes.
• Engage in strategic initiatives that promote value-based care and support enhanced patient experiences.
• Deliver high-quality customer service to foster patient satisfaction and retention rates.

• Coordinate patient care with other healthcare providers and specialists, including utilizing telemedicine resources.
• Be actively present at assigned facilities during work hours to ensure optimal resident care coverage.
• Participate in family conferences and provide essential education regarding advanced care planning.
• Address pharmacy recommendations and collaborate with pharmacists to enhance medication management for residents.

Patient Population / Client Focus

Qualifications

• Active DEA license, allowing for controlled substance prescriptions.
• Authorization to work in the United States.

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP Certificate Program graduate.

Compensation and Benefits
