Position Overview

Sun Valley Heart and Vascular is seeking a full-time Outpatient Cardiology Nurse Practitioner to join our dedicated team in Haverstraw, New York. This role offers an exciting opportunity for an NP to work within our Center for Advanced Cardiac Care (CACC), where you will function under the guidance of the Chief Nurse Practitioner and the Medical Director in the Division of Cardiology. As a vital member of our healthcare team, you will contribute to the management and treatment of patients with diverse cardiology-related conditions.

Work Environment

The position is located in our outpatient office in Haverstraw, where you will work alongside a team of experienced cardiologists, nurse practitioners, and skilled administrative staff. The work schedule is Monday through Friday from 9 am to 5 pm, freeing you from night shifts and weekend duties, allowing for a better work-life balance. In this dynamic outpatient setting, you will engage directly with patients and their families, ensuring comprehensive cardiovascular care delivery.

Key Responsibilities

• Conduct thorough patient evaluations including medical histories and physical examinations, with a focus on patients experiencing symptoms or diagnoses related to cardiac health.
• Develop and implement individualized patient care plans, managing chronic conditions such as hypertension and hyperlipidemia, while ensuring medication management is aligned with best practices.
• Provide outpatient-specific Electrophysiology services, addressing conditions such as abnormal ECGs and arrhythmias, and working closely with device patients.
• Manage acute cardiac conditions, including but not limited to Coronary Artery Disease, Congestive Heart Failure, and Vascular Heart Disease, utilizing evidence-based guidelines for treatments.
• Oversee stress testing procedures for patients, interpreting results and integrating findings into patient management strategies.
• Collaborate with cardiologists in a supervisory capacity for complex case management, ensuring optimal patient outcomes.
• Refer patients to specialists as needed and liaise with other healthcare providers to coordinate comprehensive care.
• Educate patients and their families about diagnoses, treatment options, and lifestyle modifications necessary for cardiac health improvement.

Patient Population / Client Focus

Your role will primarily involve managing adult patients within an outpatient cardiology context. This includes working with individuals diagnosed with chronic cardiovascular diseases, arranging routine follow-ups, and ensuring early intervention when required, thereby contributing significantly to improved health outcomes.

Qualifications

• A Bachelor’s Degree in Nursing or a related field from an accredited institution.
• Completion of an accredited Nurse Practitioner program with a focus on cardiology or related disciplines.
• Current certification as a Nurse Practitioner with an active license to practice in New York State.

Preferred Experience

• Hands-on clinical experience in a cardiology setting, whether outpatient or inpatient, is preferred but not mandatory.
• Experience in managing chronic cardiovascular conditions or performing procedures related to Electrophysiology would be an asset.

Compensation and Benefits
