Work Environment

This role is located in our outpatient office, where you will collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. Our teamwork approach allows you to focus on delivering direct patient care and coordination, utilizing a robust support network. The workplace is structured to facilitate direct patient interactions and care management while providing access to a network of elite specialists and hospitals for any necessary referrals.

Key Responsibilities

• Provide exceptional primary care to adult patients, utilizing your expertise to assess, diagnose, and manage a variety of health conditions.
• Conduct both routine and acute office visits, ensuring each patient receives personalized attention and care.
• Administer annual wellness visits and comprehensive health risk assessments, with a focus on precise documentation and a holistic view of their health.
• Coordinate care with other medical providers, testing facilities, specialists, and agencies to streamline patient services.
• Lead population health initiatives as part of a care team, ensuring that patients receive essential screenings and timely interventions.
• Assist with triage and patient outreach via phone, improving access to care and patient engagement.
• Educate patients about their health conditions, treatment plans, and overall well-being to foster understanding and adherence.
• Participate in promotional activities to enhance community awareness of our services and mission.
• Conduct home visits when necessary, adapting care to meet patients' unique situational needs.
• Execute additional tasks as assigned which contribute to the operational excellence of our practice.
• Report to the Medical Director while maintaining active collaboration with operational leadership.

Patient Population / Client Focus

Our primary focus is on adult patients, particularly those enrolled in Medicare. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP certification.
• Current national certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• Possession of an active, non-probationary Nurse Practitioner license in New York.
• Active DEA license.
• Prior experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Strong understanding of documentation practices, including HCC, ICD-10 coding, and Health Risk Assessments.
• A passion for collaborative healthcare, embodying teamwork in a cross-functional environment.

Preferred Experience

• Experience working with Medicare populations and familiarity with value-based care models.
• Proficiency with electronic medical records (EMR) systems for effective documentation and patient tracking.
• Previous involvement in population health initiatives or quality improvement projects.

Compensation and Benefits

This full-time role offers a competitive salary ranging from USD 120,000 to 160,000 annually, dependent on experience and qualifications.
