Full-Time Nurse Practitioner - Primary Care Specialist

Position Overview

Pulse Point Internal Medicine is seeking a dedicated and compassionate full-time Nurse Practitioner to join our esteemed healthcare team in Haverstraw, New York. In this role, you will independently diagnose and treat patients while collaborating with a network of healthcare professionals to deliver comprehensive, patient-centered primary care. You will be integral in fostering meaningful relationships with our patients, ensuring they feel connected to their care, and maintaining their health and well-being.

Work Environment

As a Nurse Practitioner at Pulse Point Internal Medicine, you will primarily work onsite in our outpatient practice environment. You will be part of a dynamic care team that includes physicians, physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. This collaborative atmosphere allows you to focus on providing excellent primary care while ensuring streamlined care coordination with various specialists and agencies.

Key Responsibilities

• Delivering exceptional primary care to patients, focusing on establishing long-term health management plans.
• Conducting thorough office visits for both routine check-ups and acute health situations.
• Administering annual wellness visits and health risk assessments, ensuring comprehensive documentation and assessments.
• Coordinating care among other health providers, specialists, testing facilities, and community resources to facilitate seamless patient care.
• Overseeing population health initiatives that may include ensuring all eligible female patients receive urgent breast cancer screenings every two years.
• Managing patient outreach through phone triage and education, ensuring patients understand their health conditions and care plans.
• Participating in community outreach and promotional events to enhance healthcare access.
• Providing home visits when necessary to assess patient health in their own environment.
• Reporting to the Medical Director and closely collaborating with operational leadership to meet organizational goals.

Patient Population / Client Focus

Our patient population primarily consists of older adults on Medicare who seek personalized primary care. This involves building trusting relationships and ensuring our patients are engaged in every aspect of their healthcare, thereby fostering better outcomes.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or a graduate of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Possession of a valid and active Nurse Practitioner state license.
• A current DEA license for prescribing medications.
• Prior experience in primary care settings, ideally focusing on internal medicine, geriatrics, or family medicine.
• Proficient understanding of Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and Health Risk Assessments.
• A strong propensity for teamwork and a passion for innovative, population-health-focused care delivery.

Preferred Experience

• Bilingual proficiency to better serve our diverse patient population.
• Experience in utilizing electronic health record (EHR) systems for patient documentation and care management.
• Familiarity with care coordination and community health resources to enhance patient access to care.

Compensation and Benefits

The annual salary for this position ranges from USD 180,000 to 200,000, commensurate with experience and qualifications. In addition, we offer a comprehensive benefits package that includes:
