Work Environment

This role is situated in the bustling and supportive environment of New City, nestled within a vibrant community that values health and wellness. You'll be working full-time, Monday through Friday, from 9 AM to 5 PM. The onsite nature of this position means you will interact directly with patients in their homes, allowing for a personalized approach to care delivery while adapting to each patient's living conditions and needs.

Key Responsibilities

• Perform in-depth health assessments and physical examinations to accurately diagnose and develop individualized treatment plans tailored to each patient’s needs.
• Order, interpret, and follow up on diagnostic tests to monitor patient progress and adjust treatment protocols accordingly.
• Prescribe medications, administer therapeutic procedures, and educate patients on their management plan, fostering understanding and compliance.
• Engage in patient education, offering guidance on health promotion and chronic disease management strategies that empower patients to take charge of their health.
• Collaborate with an interdisciplinary team, including physicians, nurses, and social workers, to ensure a seamless continuum of care and to facilitate referrals to specialists when necessary.
• Document patient interactions and care plans meticulously in electronic medical records, adhering to regulatory standards and ensuring the accuracy of health data.

Patient Population / Client Focus

As a Nurse Practitioner at Handspring Clinical Services, you will work with a diverse range of patients, from pediatric to geriatric populations. Many of our clients have chronic conditions that require regular management, including diabetes, hypertension, and cardiovascular diseases. Your role will be to build trust with patients, understanding their unique histories, and developing comprehensive, individualized care plans that enhance their overall wellbeing.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Current licensure as a Nurse Practitioner in New York State.
• Certification in a recognized Nurse Practitioner specialty area is preferred, demonstrating advanced clinical expertise.
• Exceptional clinical assessment abilities, equipped to handle various acute and chronic conditions.
• Strong communication and interpersonal skills, essential for building rapport with patients and collaborating effectively with team members.
• A proactive approach to working in a fast-paced healthcare setting and a commitment to continued learning.

Preferred Experience

• A minimum of 2 years of clinical experience as a Nurse Practitioner, ideally with a focus on home care or a similar setting.
• Experience with electronic health records (EHR) systems and other healthcare technologies that aid in patient management and data documentation.

Compensation and Benefits

This position offers a competitive salary ranging from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
