Work Environment

Key Responsibilities

• Conduct comprehensive patient assessments, including physical examinations and health history reviews, to develop individualized care plans.
• Diagnose common acute and chronic illnesses, offering timely treatment interventions that align with evidence-based practices.
• Prescribe appropriate medications and develop detailed treatment plans tailored to each patient’s unique health status.
• Administer immunizations and vaccinations following established guidelines to prevent the spread of preventable diseases.
• Provide thorough counseling regarding health promotion strategies, disease prevention methods, and necessary lifestyle adjustments.
• Order and interpret essential diagnostic tests, ensuring accurate understanding and follow-up on results with patients and their families.
• Collaborate with multidisciplinary healthcare professionals including social workers, nurses, and physicians to ensure seamless patient care coordination.
• Maintain meticulous and up-to-date electronic medical records, documenting all patient encounters with precision and clarity.

Patient Population / Client Focus

Qualifications

• Education: A Master's degree in Nursing from an accredited program is required.
• Licensure: An active Nurse Practitioner license in the state of New York is mandatory.
• Certification: Certification as a Family Nurse Practitioner is essential for this role.

Preferred Experience

• A minimum of 2 years of clinical experience as a Nurse Practitioner, focusing on family health across age groups, specifically including experience with children and elder care.
• Experience in home care services or community health settings is highly preferred.

Compensation and Benefits

This role offers a competitive salary ranging from $150,000 to $160,000 annually, commensurate with experience and qualifications.
