This full-time, onsite position offers an annual salary range of $180,000 to $200,000, contingent on qualifications and experience. Key Responsibilities

• Conduct comprehensive health assessments, including physical examinations and health screenings, to identify individuals' needs.
• Diagnose and manage both acute and chronic health conditions, administering preventative care and health promotion strategies tailored to individual and family needs.
• Collaborate closely with local community organizations, healthcare teams, and stakeholders to design, develop, and implement engaging health education programs.
• Deliver culturally competent care that respects the diverse backgrounds of patients, while advocating for their rights and access to required healthcare services.
• Coordinate patient referrals and follow-up care, ensuring continuity and quality of care through meticulous planning and communication.
• Maintain detailed and accurate patient records in compliance with healthcare regulations and standards, utilizing electronic health record (EHR) systems.
• Participate in community outreach events and health fairs, demonstrating our commitment to raising health awareness and providing essential screenings and services.

Patient Population / Client Focus

In this role, you will primarily serve individuals and families residing in assisted living settings. Qualifications

• Current certification as a Nurse Practitioner.
• A valid nursing license issued by the State of New York.
• A Master's degree in nursing from an accredited institution.

Compensation and Benefits
