Work Environment

This onsite position is based at our outpatient office in the vibrant Spring Valley area, known for its rich culture and community spirit. Here, Nurse Practitioners collaborate closely with a multidisciplinary team, including physicians, nurses, and administrative staff, in a dynamic and fast-paced healthcare environment. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be part of your responsibilities, contributing to a total average workweek of approximately 40 hours.

Key Responsibilities

• Perform comprehensive health assessments and screenings tailored to individuals and families, including collecting detailed patient histories and health data.
• Diagnose and manage a variety of acute and chronic medical conditions, utilizing evidence-based practices to engage in preventive care and health promotion strategies.
• Develop and oversee health education programs in collaboration with community organizations, focusing on prevalent health issues within the population.
• Advocate for patients by ensuring culturally sensitive care and championing their rights and access to healthcare services.
• Coordinate referrals and follow-up care, establishing continuity in the patient’s treatment journey and ensuring their needs are met.
• Maintain accurate and detailed medical records in compliance with regulatory standards and best practices in documentation.
• Engage in outreach initiatives and health fairs designed to raise awareness of health issues and provide community-wide health screenings.
• Stay informed about community health trends, healthcare policies, and advances in clinical practice to continually enhance care delivery.

Patient Population / Client Focus

You will be working primarily with a diverse patient population in the Spring Valley area, where healthcare disparities exist. Your role will address the unique needs of various communities, requiring cultural competency and an understanding of social determinants of health. This role plays a crucial part in promoting health equity and empowering individuals through education and support.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by recognized bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active and unrestricted Nurse Practitioner (APRN) license in New York State.
• Valid DEA and NPI numbers are required.
• Strong clinical assessment and diagnostic skills are essential for this role.
• Excellent communication abilities and interpersonal skills to foster patient relationships.
• Proficiency with electronic health records (EHR) systems for effective documentation and patient tracking.

Preferred Experience

• A minimum of 2 years of clinical experience in a primary care or family medicine setting.
• Experience working in community health or public health environments is highly desirable.
• Proven ability to work both independently and as part of a collaborative healthcare team.

Compensation and Benefits
