Work Environment

As part of our team, you'll be working onsite at our hospital facility, where a supportive and collaborative environment awaits you. The clinic operates Monday through Friday, from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. This position encompasses an average workweek of around 40 hours, with the possibility of participating in community outreach events or health fairs during weekends, ensuring a flexible work-life balance.

Key Responsibilities

• Conduct thorough health assessments for individuals and families, identifying their specific healthcare needs and creating personalized care plans.
• Diagnose and manage a variety of acute and chronic medical conditions, offering preventive care services that improve patient health outcomes.
• Engage with local community organizations to design and implement health education initiatives aimed at raising awareness and promoting healthy practices within the community.
• Deliver culturally sensitive care, advocating for the rights of patients and ensuring their access to healthcare services is upheld.
• Coordinate referrals and follow-up care to maintain continuity of treatment, leveraging our electronic health record system to track and communicate patient progress.
• Maintain detailed medical documentation in accordance with regulatory standards, ensuring accuracy and compliance with best practices in patient care.
• Participate in community health fairs and outreach events to provide screenings and educational services that meet the immediate needs of the community.
• Stay informed about emerging trends in community health and incorporate evidence-based practices into care delivery for optimal patient outcomes.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license.
• Completion of a Master's degree in nursing from an accredited program.

Preferred Experience

Compensation and Benefits
