Work Environment

Key Responsibilities

In your capacity as a Nurse Practitioner, your responsibilities will include:

• Conducting comprehensive cardiovascular assessments and screenings for residents, utilizing advanced diagnostic tools and techniques to ensure accurate evaluation.
• Diagnosing and managing a variety of acute and chronic heart conditions, developing personalized treatment plans that address specific patient needs and preferences.
• Providing education on heart health and preventive care, empowering patients and families to make informed lifestyle choices that promote cardiovascular well-being.
• Collaborating with community organizations and stakeholders to design and implement educational programs that elevate awareness about heart health.
• Maintaining detailed and compliant medical records in accordance with regulatory standards to facilitate coordination and continuity of care.
• Staying current with the latest cardiovascular health trends, evidence-based practices, and healthcare policies to ensure that patient care remains at the forefront of best practices.

Patient Population / Client Focus

Your primary focus will be on older adults living in the assisted living facility, many of whom may have multiple comorbidities and varying degrees of cardiovascular health challenges. You will be instrumental in providing culturally sensitive care and advocating for their rights and needs, ensuring they have equitable access to necessary cardiology services. Your role will also involve engaging with families to enhance their understanding and involvement in their loved ones’ care.

Qualifications

• A Master’s degree in Nursing from an accredited program, with specialization in Cardiovascular Nursing or a related field.
• Active nursing license in New York State, with certification in Cardiovascular Nursing preferred.
• A minimum of 3 to 5 years of clinical experience as a Nurse Practitioner, ideally in a cardiology or cardiovascular health setting.

Preferred Experience

Experience working in community health or public health settings is highly valued. Familiarity with geriatric care and previous involvement in health promotion or disease prevention programs will give you an advantage in this role.

Compensation and Benefits

We offer a competitive salary ranging from $150,000 to $160,000 per year, commensurate with experience, along with a comprehensive benefits package that includes:
