Primary Care Nurse Practitioner in Assisted Living Facility

This full-time position requires a passionate healthcare professional to provide essential primary and specialty care within an assisted living facility. Work Environment

This role is onsite at our assisted living facility in Spring Valley, where you will work closely with a multidisciplinary healthcare team. The facility promotes a vibrant community atmosphere, focused on the holistic well-being of its residents. You can expect a collaborative environment, equipped with state-of-the-art medical resources, ensuring that you can deliver optimal care. Operating hours are Monday through Friday, from 9 AM to 5 PM.

Key Responsibilities

• Conduct thorough physical examinations and comprehensive health assessments to establish diagnoses for our elderly residents, tailoring treatment plans to fit individual needs.
• Order, analyze, and interpret diagnostic tests while prescribing appropriate medications and executing therapeutic procedures relevant to identified health conditions.
• Provide patient education and counseling on health maintenance, disease prevention, and management of chronic health issues including diabetes, hypertension, and heart disease.
• Work alongside physicians and other nursing staff, ensuring seamless care coordination, tracking referrals, and maintaining continuity of care throughout the treatment process.
• Document all encounters accurately and in accordance with regulatory standards, utilizing our electronic health record system to maintain up-to-date patient information.

Patient Population / Client Focus

In this role, you will primarily care for elderly patients residing in the assisted living facility, addressing a broad spectrum of healthcare challenges typical of aging populations. This includes multi-morbidity management, rehabilitation care, and end-of-life discussions, all tailored to enhance residents' quality of life.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Current state licensure as a Nurse Practitioner.
• Certification in a recognized Nurse Practitioner specialty area is preferred.
• Strong clinical assessment skills and proficiency in diagnosing acute and chronic conditions inherent to geriatric care.
• Excellent communication skills in order to interact effectively with patients, families, and healthcare coworkers.
• Strong organizational capabilities to manage multiple patient needs in a fast-paced environment.

Preferred Experience

• At least two years of direct clinical experience as a Nurse Practitioner, preferably in a geriatric or long-term care setting.
• Experience in working with electronic health record systems for documentation and patient management.
• Demonstrated ability to work collaboratively as part of a healthcare team.

Compensation and Benefits

The compensation for this full-time role ranges from $150,000 to $160,000 annually.
