Work Environment

This role is based onsite within a diverse range of healthcare settings, including private residences, assisted living facilities, and skilled nursing facilities. Key Responsibilities

• Collaborate with local and regional operations, and clinical leadership teams to align on healthcare goals and best practices.
• Support and implement strategic initiatives focused on value-based care to improve patient outcomes.
• Deliver an exceptional customer experience while ensuring high levels of patient satisfaction and retention through attentive care.
• Practice within ethical boundaries and adhere to the Scope and Standards of Practice pertinent to your certification and professional background.
• Ensure compliance with all state and federal regulations, including the guidelines set forth by the local healthcare authority.
• Work closely with primary care providers (PCPs) and the interdisciplinary care team to deliver seamless patient care at assigned facilities.
• Coordinate and manage communication with other healthcare providers and specialists, including telehealth professionals to provide comprehensive care.
• Conduct all necessary monthly, quarterly, annual visits, and acute visits according to regulatory requirements.
• Engage in family conferences, providing advanced care planning, educational resources, and support to both patients and families.
• Participate actively in addressing pharmacy recommendations and collaborate with pharmacists to optimize patient therapeutic outcomes.
• Be physically present in assigned facilities during working hours to ensure adequate face-to-face coverage that meets both resident care and facility demands.
• Assess, diagnose, and manage an array of chronic and acute medical conditions, tailoring treatment plans effectively to each patient's needs.
• Provide valuable health education to patients and families, fostering a collaborative approach to wellness.
• Order and interpret appropriate lab tests and diagnostic studies, modifying treatment plans based on results and patient feedback.
• Maintain accurate and compliant documentation using our Electronic Medical Record (EMR) systems to ensure continuity of care.

Qualifications

• Must possess US work authorization.
• Flexibility to work in a dynamic environment and adapt to various scheduling requirements.
• A strong passion for providing high-quality healthcare services to senior citizens and economically disadvantaged individuals.
• A Master of Science in Nursing, Doctor of Nursing Practice, or equivalent recognition through an NP certificate program.
• National certification through AANP (American Association of Nurse Practitioners) or ANCC (American Nurses Credentialing Center).
• Active, unrestricted nursing license in the state of New York.
• Active DEA license.

Preferred Experience

• Experience in concierge medicine or geriatric care preferred but not mandatory.
• Previous work in interdisciplinary team environments is an advantage.
• Familiarity with EMR systems, especially those specific to the healthcare setting.
