Key Responsibilities

• Conduct comprehensive assessments by gathering health histories and performing physical examinations to evaluate the cardiovascular health of patients.
• Develop and implement individualized treatment plans based on clinical findings and laboratory results, including ordering and interpreting diagnostic tests, labs, and imaging studies.
• Prescribe medications and therapeutic interventions following established guidelines to manage acute and chronic cardiovascular conditions.
• Educate patients and their families on cardiovascular disease management, health promotion, and preventive care strategies tailored to each patient's unique circumstances.
• Engage in interprofessional collaboration with cardiologists, nurses, and ancillary staff to ensure continuity of care and optimal patient outcomes.
• Mentor and support newer nursing staff and students in cardiology practices and protocols as a role model of clinical excellence.

Patient Population / Client Focus

In this role, you will primarily interact with elderly patients residing in an assisted living facility, many of whom have multiple comorbidities. Your expertise in cardiology will enable you to provide specialized care for patients experiencing a range of cardiovascular conditions, enhancing their overall health through a compassionate and proactive approach.

Qualifications

• Master's degree in Nursing required.
• Current Nurse Practitioner licensure in New York State.
• Strong clinical competency in cardiology and experience in outpatient or inpatient healthcare settings.

• A minimum of 2 years of clinical practice experience as a Nurse Practitioner, with a preference for those who have worked in cardiology or a closely related specialty.
