Work Environment

The Nurse Practitioner will be immersed in a supportive and collaborative work environment situated within a vibrant Assisted Living Facility. Our team consists of dedicated healthcare professionals committed to providing high-quality medical care and supportive services. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and participation in community health outreach events on weekends may be required. An average workweek is approximately 40 hours, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct thorough health assessments and routine screenings for residents and community members to identify healthcare needs and risks.
• Diagnose and manage acute and chronic conditions, while emphasizing preventive care and health education tailored to individual needs.
• Collaborate with local organizations and healthcare stakeholders to develop, implement, and evaluate community health education programs.
• Advocate for patient rights and ensure culturally responsive care, fostering a welcoming environment for all patients.
• Coordinate referrals and follow-up care, ensuring seamless transitions for patients between different levels of care.
• Maintain accurate and comprehensive medical records in line with regulatory requirements and best practices in documentation.
• Engage in outreach initiatives and health fairs, leveraging these platforms to provide health screenings and increase community health awareness.
• Stay informed on emerging community health trends, evidence-based practices, and relevant healthcare policies to continuously improve service delivery.

Qualifications

• Current certification as a Nurse Practitioner from an accredited institution, demonstrating expertise in clinical practice.

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
