Work Environment

This position is onsite at our outpatient office, where you will find a supportive and collaborative team. Your work schedule will typically be Monday through Friday, from 9:00 a.m. to 6:00 p.m., with opportunities for occasional evening clinics until 8:00 p.m. Participation in community events and health fairs may occur on weekends, broadening our outreach and impact in the community. The average workweek will consist of approximately 40 hours, allowing you to balance professional dedication with personal time.

Key Responsibilities

As a Community-Focused Nurse Practitioner, you will have a diverse range of responsibilities:

• Conduct thorough health assessments for individuals and families, capturing detailed medical histories and identifying health risks.
• Diagnose and manage both acute and chronic medical conditions, implementing preventive care strategies and health promotion activities tailored to community needs.
• Develop and facilitate community health education programs in collaboration with local organizations, informing the public about wellness, disease prevention, and available resources.
• Provide culturally competent care, advocating fervently for your patients' rights and securing their access to services they require.
• Coordinate referrals and ensure follow-up care, maintaining continuity in the patient’s health journey.
• Document all patient interactions and care plans accurately in compliance with legal and ethical standards, using electronic health record systems.
• Engage in active community outreach by participating in health fairs and local events to raise awareness and provide necessary health screenings.
• Stay informed on community health trends, best practices, and relevant health policies to enhance service delivery.

You will primarily serve diverse individuals and families in the Yorktown Heights area, including those from underserved and vulnerable populations. Qualifications

To qualify for this position, you must meet the following criteria:

• Active Nurse Practitioner certification.
• Current state nursing license in New York.
• A master’s degree in nursing from an accredited institution.

Preferred Experience

Compensation and Benefits

This rewarding position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
