Work Environment

This full-time Nurse Practitioner role is onsite at our Assisted Living Facility, where you will work collaboratively with a diverse team of healthcare professionals. Our work schedule typically spans from 9:00 a.m. to 6:00 p.m., with potential for evening clinics extending until 8:00 p.m. You may also participate in community outreach events or health fairs during weekends, contributing to our goal of health education and promotion within the community. Key Responsibilities

• Conduct thorough health assessments and screenings, developing personalized care plans for both acute and chronic medical conditions.
• Diagnose illnesses and prescribe appropriate medications while monitoring patient responses to treatment regimes.
• Facilitate health education initiatives in collaboration with community stakeholders, sharing insights about disease prevention and management.
• Ensure cultural competency in care delivery, advocating for patient rights and equitable access to health services.
• Coordinate referrals and follow-up care, maintaining continuity of care to enhance patient outcomes and treatment effectiveness.
• Maintain all medical documentation accurately and in compliance with local, state, and federal regulations, ensuring high-quality record-keeping practices.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner with an active nursing license in New York State.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, reflecting your experience and qualifications.
