Work Environment

Located in the heart of Ossining, our outpatient office is designed to foster a collaborative and supportive atmosphere among our healthcare providers. You will work onsite, directly within a multidisciplinary team that values respect, compassion, and teamwork. The standard work hours are typically from 9:00 a.m. to 6:00 p.m., with occasional late clinics until 8:00 p.m. and weekends dedicated to community outreach events or health fairs.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages, identifying their unique healthcare needs and providing personalized, evidence-based care.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and promoting health education to facilitate better health outcomes.
• Engage with community organizations to develop and implement targeted health education programs, ensuring these initiatives meet the specific needs of the populations served.
• Provide culturally sensitive care that respects the diverse backgrounds and values of our patients, advocating for their rights to access quality healthcare services.
• Coordinate referrals and promote continuity of care by managing follow-up appointments and communications between healthcare providers.
• Maintain detailed and accurate medical records in accordance with regulatory standards and organizational policies, ensuring confidentiality and compliance.
• Participate actively in community outreach events and health fairs, increasing health awareness and providing vital health screenings to those who may not regularly access healthcare services.
• Stay current with community health trends, best practices, and relevant healthcare policies to continually improve care delivery.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.

Compensation and Benefits
