Work Environment

The work environment is collaborative and supportive, with a focus on teamwork and continuous professional development. As a Nurse Practitioner in our hospital setting, you will engage closely with physicians, nurses, and other healthcare providers to ensure a comprehensive approach to patient care. The typical work schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. and participation in community health events during weekends expected.

Key Responsibilities

• Conduct detailed health assessments and screenings for patients within the hospital and community settings.
• Diagnose and manage both acute and chronic medical conditions, implementing tailored treatment plans that emphasize preventive care.
• Collaborate with community organizations to develop and participate in health education programs that target prevalent health issues.
• Advocate for patients' rights, striving to ensure that every individual has fair access to healthcare services.
• Coordinate referrals and follow-up care, ensuring patient continuity and support through their healthcare journey.
• Maintain accurate and comprehensive medical records in compliance with healthcare regulations and organizational standards.
• Stay informed about current community health trends and evidence-based practices to enhance care delivery and patient education.

Patient Population / Client Focus

The focus population includes individuals and families within the Cortlandt Manor area, particularly those from underserved communities who face barriers to accessing quality healthcare. You will play a vital role in addressing these barriers, improving health literacy, and empowering patients to take an active role in their health management.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA and NPI numbers are required.
• Strong clinical assessment and diagnostic skills along with excellent interpersonal and communication abilities.
• Proficiency in the use of Electronic Health Records (EHR) systems.
• Minimum of 3–5 years of clinical experience, preferably in community health or primary care settings.

Preferred Experience

While experience in family medicine or primary care is preferred, we also encourage candidates who have a strong background in community health initiatives and public health outreach. Familiarity with cultural competency in client interactions can vastly improve patient rapport and outcomes.

Compensation and Benefits

• The salary range for this position is competitive, between $120,000 and $130,000 annually, commensurate with your experience and qualifications.
