Work Environment

This position is based onsite within a school facility, fostering a collaborative atmosphere with educators, parents, and community health resources. You will have a fixed work schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics lasting until 8:00 p.m. As a family nurse practitioner, you may also participate in community outreach events and health fairs that may take place on weekends, ensuring that you engage directly with the populations you serve. Expect a dynamic and supportive environment, averaging about 40 hours per week.

Key Responsibilities

• Conduct comprehensive health assessments, including physical exams and health screenings for students and their families to identify health issues and risks.
• Diagnose and manage both acute and chronic conditions, making informed decisions on preventive care strategies to promote wellness within the school community.
• Implement and evaluate health education programs tailored to the needs of students, working collaboratively with teachers and parents to optimize student health outcomes.
• Advocate for patients' rights and navigate healthcare access issues, providing culturally sensitive care that considers the diverse background of students and families.
• Coordinate referrals and ensure continuity of care through diligent follow-up and collaboration with other healthcare providers.
• Maintain accurate and up-to-date electronic medical records following all regulatory compliance standards, ensuring patient privacy and data protection.
• Engage in community outreach activities that promote health awareness, offering health screenings and educational opportunities to underserved populations.
• Stay informed on the latest community health trends and relevant public health policies to inform practices and adapt programs as needed.

Patient Population / Client Focus

Your role as a Family Nurse Practitioner will primarily involve working with students aged K-12 and their families, addressing diverse health needs that may include chronic conditions, mental health concerns, and preventive care. You will be instrumental in fostering a supportive environment that nurtures the overall well-being of students, thereby contributing to their academic success and personal growth.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license in New York.
• A Master's degree in nursing from an accredited institution is required.

Compensation and Benefits
