Work Environment

The role is based onsite at our state-of-the-art Assisted Living Facility, where you will work in a collaborative environment alongside a multidisciplinary team of healthcare professionals. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and may involve participation in community health outreach activities on weekends. This role is pivotal in bridging the gap for patients needing comprehensive and coordinated care.

Key Responsibilities

• Perform thorough health assessments and screenings, utilizing tools like electronic health records (EHR) to document patient information accurately.
• Diagnose and manage a spectrum of acute and chronic medical conditions, focusing on preventive care and promoting optimal health through tailored health plans.
• Collaborate with local stakeholders and community organizations to design and implement impactful health education programs tailored to the specific needs of our population.
• Deliver culturally sensitive care, ensuring that you are advocating for patients’ rights and access to necessary healthcare services.
• Coordinate and manage referrals, ensuring seamless follow-up care, which is essential for maintaining continuity of treatment.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner.

Compensation and Benefits

• We offer a competitive annual salary range of $180,000 to $200,000, commensurate with your experience and qualifications.
