Work Environment

In this full-time role, you will work primarily in a home care setting, providing personalized care directly to patients in their residences. Our collaborative environment allows our healthcare professionals to truly impact their patients' lives, tackling not only immediate health concerns but also long-term wellness strategies. Your work hours will generally run from 9:00 a.m. to 6:00 p.m., with flexibility for evening clinics until 8:00 p.m. and the possibility of weekend community outreach events.

Key Responsibilities

• Conduct thorough health assessments, including physical examinations and diagnostic tests, to develop individualized care plans for patients.
• Diagnose and manage both acute and chronic medical conditions, ensuring that patients receive appropriate preventive care and follow-up treatment.
• Engage with community organizations to design and implement effective health education initiatives tailored to the populations we serve.
• Advocate for patients’ rights ensuring they have access to healthcare resources, navigating any barriers that may impede their healthcare journey.
• Ensure continuity of care by coordinating referrals to specialists as needed, and following up on patients’ progress and health outcomes.
• Maintain comprehensive and accurate medical records in accordance with healthcare regulations and best practices, utilizing electronic health record systems to document patient encounters.
• Participate actively in community health fairs and outreach activities, providing screenings and health education to enhance community engagement.
• Stay informed on current health trends, evidence-based practices, and policies to enhance service delivery and patient outcomes.

Patient Population / Client Focus

Our clinic primarily serves a diverse patient demographic within the Yorktown Heights community. You will be working with a broad range of patients including children, adults, and families, each with unique health needs. This position necessitates the provision of culturally sensitive care, ensuring that treatments are appropriate and tailored to each patient's background.

Qualifications

• Must possess current certification as a Family Nurse Practitioner (FNP) and an active state nursing license.
• A Master's degree in Nursing from an accredited program is essential for candidates.
• Proven clinical experience of a minimum of 3 to 5 years, preferably in community health or public health settings.
• Exceptional interpersonal and communication abilities to work effectively with a diverse range of patients and stakeholders.

Compensation and Benefits

• Competitive salary ranging from USD 180,000 to 200,000 per year, based on experience and qualifications.
