Key Responsibilities

• Conduct thorough and comprehensive patient assessments, including detailed medical histories, physical examinations, and necessary diagnostic tests to evaluate health conditions.
• Collaborate with physicians and other healthcare professionals to develop and implement evidence-based treatment plans tailored to each patient’s needs.
• Provide primary and specialty healthcare services, managing acute and chronic illnesses through diagnosis and effective treatment plans.
• Deliver education and counseling to patients and their families on topics such as health promotion, disease prevention, and self-care strategies, empowering them to take an active role in their health.
• Engage in regular multidisciplinary team meetings to discuss patient cases, coordinate care efforts, and share insights on best practices to enhance patient outcomes.
• Maintain accurate and up-to-date patient medical records using electronic health record (EHR) systems, ensuring compliance with all relevant regulations and organizational policies.

Patient Population / Client Focus

Your role as a Nurse Practitioner will involve working with a diverse patient population, including individuals from various backgrounds and age groups, addressing a wide array of health issues. Qualifications
