We are seeking a dedicated and compassionate Nurse Practitioner to join our team at One Medical Health Care Clinic in Ossining, New York. Work Environment

The position is onsite at our outpatient office in Ossining, a vibrant community where you will engage directly with patients in a welcoming environment. Your work schedule will consist of Monday to Friday from 9 AM to 5 PM, allowing for a balanced professional experience. As part of a collaborative healthcare team, you will work closely with other healthcare providers to ensure the highest standards of patient care.

Key Responsibilities

• Conduct comprehensive health assessments and screenings that evaluate the physical, emotional, and social needs of patients and their families.
• Diagnose and manage a wide range of acute and chronic medical conditions, emphasizing preventive care and health promotion strategies tailored to the community.
• Work with local organizations and stakeholders to develop and lead impactful health education programs that inform and empower community members.
• Provide culturally sensitive and equitable care, advocating vigorously for patients' rights and accessible healthcare services.
• Coordinate referrals and follow-up care to ensure a seamless transition across care settings, optimizing the patient’s healthcare journey.
• Maintain precise and comprehensive medical records in compliance with regulatory standards, ensuring confidentiality and quality control.
• Engage in community outreach events and health fairs to increase awareness and accessibility of healthcare services, conducting health screenings where necessary.
• Stay informed about emerging health trends, evidence-based practices, and relevant policy changes to better serve our patient population.

Patient Population / Client Focus

The typical patient population consists of individuals and families from diverse cultural backgrounds in the Ossining area. Our clinic specifically focuses on underserved communities, striving to reduce healthcare disparities and improve health literacy. As a Nurse Practitioner, you will not only treat patients but also mentor them in achieving health equity.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a DEA number and NPI number is required for this role.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills for effective interaction with patients and team members.
• Ability to work independently while also functioning effectively as part of a multidisciplinary team.
• Proficient in using electronic health records (EHR) systems for documentation and patient management.

Preferred Experience

• Minimum of 2-5 years of clinical experience, with a preference for experience in primary care or family medicine, particularly within community health settings.
• Demonstrated ability to work effectively with diverse populations, understanding the unique challenges they face.
