This full-time position focuses on providing outstanding primary care to our patients, primarily older adults on Medicare. This position is based onsite, where you will be part of a collaborative care team that includes physicians, Nurse Practitioners, Physician Assistants, medical assistants, and dedicated support staff such as medical scribes and care managers. Key Responsibilities

• Provide exceptional primary care, managing routine and acute health issues, ensuring thorough documentation and adherence to best practices.
• Conduct annual wellness visits and health risk assessments, which emphasize a holistic approach to patient health.
• Lead care coordination efforts with specialists, testing facilities, and other healthcare providers to create integrated treatment plans.
• Champion population health initiatives, such as ensuring regular screening for diseases like breast cancer in eligible patients.
• Engage in phone triage and patient outreach, addressing concerns and providing education on health conditions.
• Participate in promotional activities to enhance patient engagement and awareness of services.
• Conduct home visits when necessary, ensuring continuity of care for our patients.
• Collaborate closely with the Medical Director and operational leadership, contributing to the overall success of the organization.

Patient Population / Client Focus

The primary focus of our services revolves around older adults on Medicare, whom we aim to keep healthy and engaged in their lifestyles. Qualifications

• A Master of Science in Nursing or Doctor of Nursing Practice, or equivalent NP Certificate Program.
• National Nurse Practitioner certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of a valid, non-probationary Nurse Practitioner license in New York State.
• An active DEA license to prescribe medications.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Preferred Experience

• A passion for collaborative teamwork and a desire to innovate within a progressive primary care model.
• Bilingual abilities are a plus, as they enhance communication with diverse patient populations.

Compensation and Benefits

We offer a competitive salary ranging from $180,000 to $200,000 per year, commensurate with experience and expertise. In addition, our comprehensive benefits package includes:
