Work Environment

This role operates onsite, primarily within the patients' homes, allowing for a flexible and dynamic working environment. Key Responsibilities

• Conduct comprehensive health assessments, taking detailed histories and performing physical examinations to inform clinical decision-making.
• Collaborate with the healthcare team to develop individualized care plans based on sound clinical judgments and patient preferences.
• Maintain accurate and up-to-date records of nursing care plans, progress notes, and interventions using electronic health record (EHR) systems to ensure continuity of care.
• Educate patients and their families about conditions, treatment options, and preventive measures to empower them in managing their health.
• Engage in the evaluation of nursing practices and procedures, providing feedback and identifying areas for improvement.
• Participate in professional development activities, including continuing education to cultivate advanced clinical skills and stay abreast of nursing practices.

Patient Population / Client Focus

The nurse practitioner will serve a diverse patient population, including elderly individuals, those with chronic illnesses, and patients requiring preventative health measures. Our focus on concierge medicine means that you will provide personalized attention, often developing long-term relationships with patients to ensure their health and wellness goals are met effectively.

Qualifications

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or equivalent qualification.
• National certification from AANP or ANCC is required.
• Active, unrestricted Nurse Practitioner license in the state of New York.
• Proof of U.S. work authorization.
• Demonstrated ability to work within a flexible schedule to accommodate patient needs.

Preferred Experience

• Prior experience in concierge medicine or home healthcare is highly valued.
• Experience with geriatric patients or chronic disease management.
• Proficient with EHR systems and telehealth platforms.

Compensation and Benefits

The salary for this full-time Nurse Practitioner role ranges from USD 150,000 to USD 160,000 annually, commensurate with experience and caseload.
