Work Environment

This role is primarily onsite, as it involves making house calls to patients in various localities around Cortlandt Manor. The Nurse Practitioner will operate in a dynamic and fast-paced environment, adapting to different patient needs and situations. Unlike standard healthcare settings, our concierge model emphasizes flexibility and personalized care, allowing the NP to have deeper interactions with patients and provide tailored treatment.

Key Responsibilities

• Conduct comprehensive physical examinations to assess patients' health and wellness, focusing on acute, episodic, or chronic conditions.
• Diagnose and treat illnesses, prescribing necessary medications and therapies as appropriate.
• Facilitate referrals to specialists and coordinate with members of the healthcare team to ensure streamlined patient care.
• Communicate effectively with collaborating physicians and nursing staff, implementing patient management policies and procedures.
• Build rapport with patients, ensuring they feel comfortable discussing their health concerns in a supportive environment.
• Manage a diverse caseload, ensuring that each patient receives the attention and care they deserve.
• Perform additional duties as assigned, demonstrating flexibility and a proactive approach to patient care.

Patient Population / Client Focus

Our Nurse Practitioner will primarily serve senior and low-income populations, including those who may have limited access to traditional healthcare settings. This role requires a genuine passion for providing equitable healthcare and understanding the unique challenges faced by these communities. The NP will play an essential role in bridging gaps in healthcare access, ensuring patients receive timely and appropriate medical attention.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner program.
• Active DEA license and unrestricted state license, with authorization to practice in New York.
• National certification from recognized bodies such as the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Excellent written and verbal communication skills, with the ability to convey complex information clearly to patients and families.
• Strong clinical judgment and the ability to work independently in a fast-paced environment.

Preferred Experience

• Previous experience in a concierge medicine or home healthcare setting is highly valued.
• Proficiency in Microsoft Office Suite, as this will facilitate documentation and communication with internal teams.
• Demonstrated ability to manage patient relationships and navigate healthcare systems effectively.

Compensation and Benefits
