Work Environment

This position is an onsite role, which emphasizes hands-on interactions with patients within their homes and at facilities such as skilled nursing facilities, independent living homes, and assisted living centers. Key Responsibilities

• Maintain comprehensive and compliant documentation using our electronic medical record (EMR) system, ensuring accuracy and accessibility for all team members involved in patient care.
• Collaborate closely with local and regional clinical leadership, aligning the practice’s goals with broader organizational targets related to patient health outcomes and quality improvements.
• Support strategic initiatives that focus on value-based care, providing patients with tailored healthcare solutions that combat chronic illnesses and promote well-being.
• Deliver an elevated customer experience, actively engaging with patients and their families to ensure satisfaction and build long-term relationships.
• Practice with integrity and adhere strictly to the Scope and Standards of Practice as well as Board Certification requirements to uphold the profession’s ethical standards.
• Comply with all pertinent state and federal regulations, ensuring that our practice operates within the legal healthcare framework.

Patient Population / Client Focus

You will primarily care for an elderly population, providing services that adapt to their unique healthcare needs and preferences. This may include managing chronic health conditions, performing wellness checks, and addressing urgent medical concerns through house calls and in-facility visits. Your role is essential in promoting health education, advocating for preventive care, and guiding patients through changes in their health status.

Qualifications

• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, unrestricted Nurse Practitioner license for the state of New York.
• Authorization to work in the United States.
• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or equivalent NP Certificate Program credentials.
• Exceptional ability to adapt to a flexible work environment, embracing various schedules as dictated by patient needs.

Preferred Experience

• Experience working in concierge medicine or delivering house call services is highly advantageous.
• Prior involvement in value-based care initiatives or similar healthcare models is advantageous.

Compensation and Benefits

• Annual salary range between $150,000 and $160,000 , dependent on patient caseload and experience.
