Work Environment

Key Responsibilities

• Develop and implement patient-centered care plans that encompass various modalities including medications, nutrition, physical activity, and standard nursing interventions to address both wellness and illness.
• Collaborate with a multidisciplinary team, including physicians and specialists, to coordinate comprehensive patient care and establish referral pathways as needed.
• Conduct thorough evaluations and document patient responses to treatment plans while maintaining accurate records per healthcare regulations.
• Provide telephone consultations and manage triage for walk-in patients, directing them to the appropriate providers based on assessed needs.
• Diagnose health conditions and prescribe treatments based on comprehensive assessments and established clinical guidelines.
• Act as a clinical resource for other healthcare providers and support staff, helping to elevate the standard of care across the facility.
• Participate in training and mentoring new nurse practitioners, fostering an environment of professional development and growth.
• Contribute to the development and periodic review of NP-specific practice guidelines, ensuring they reflect current best practices.
• Engage in regular audits of peer practices to uphold quality standards and continuously improve patient care.
• Maintain strict confidentiality regarding student health information and uphold a sensitive approach to patient interactions.
• Deliver direct patient care during clinical sessions, conducting health histories and specific examinations aligned with patient needs.
• Order and interpret diagnostic tests as necessary to inform treatment decisions.
• Identify and manage both acute and chronic health issues in adherence to approved clinical protocols.

Patient Population / Client Focus

You will primarily serve children and adolescents within a school setting, addressing a diverse array of health issues that are prevalent in this age group. This includes both acute conditions—like injuries and infections—and chronic challenges—such as asthma and diabetes management. Your role is crucial in promoting preventative health measures and educating students about maintaining a healthy lifestyle.

Qualifications

• A current Nurse Practitioner (NP) license in New York State, with certification in Family Health or Adult Health.
• A Master's Degree in Nursing from an accredited institution.
• A valid NPI number and DEA certification.
• Strong skills in communication, diagnosis, and patient management.

Preferred Experience

Ideal candidates will possess a minimum of three years of clinical experience in ambulatory care settings. This experience should include working with children and families in various capacities, ideally within educational or community health contexts. Familiarity with electronic health records (EHR) systems and patient care management software is preferred to streamline patient documentation and data management.

Compensation and Benefits

The compensation for this role ranges from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
