Work Environment

This full-time role is conducted onsite, primarily within our patients' homes across the Cortlandt Manor area. Key Responsibilities

• Conduct comprehensive assessments of patients, including gathering health histories and conducting physical examinations.
• Utilize established clinical protocols to diagnose and manage acute and chronic medical conditions.
• Develop individualized care plans in collaboration with patients and families, focusing on realistic health goals.
• Educate patients and their families regarding preventive care, medication management, and health maintenance strategies.
• Provide timely follow-ups and reassessments to ensure continuity of care and adjust treatment plans as necessary.
• Document all interactions and care plans in the electronic health record (EHR) system, ensuring compliance with health regulations.
• Participate in quality improvement initiatives aimed at improving patient outcomes and health equity within the community.

Patient Population / Client Focus

The ideal candidate will have a strong passion for working with diverse populations, particularly within underserved communities. You will encounter a variety of patient needs, including chronic illnesses, mobility issues, and mental health concerns. Understanding the social determinants of health will help tailor care to benefit each patient's unique circumstances.

Qualifications

• Hold a Master’s degree in Nursing from an accredited institution.
• Possess current certification as a Nurse Practitioner and an active nursing license in New York.
• Demonstrate at least 3 years of clinical experience, ideally within home care, community health, or public health settings.
• Exhibit strong interpersonal, communication, and collaboration skills, particularly with diverse populations.
• Show a commitment to advocating for vulnerable communities and addressing health disparities.

Preferred Experience

• Experience in home health care or a similar environment is highly desirable, as it will provide a foundational understanding of patient-centered care delivery in home settings.
• Familiarity with telehealth services and the use of telecommunication tools to enhance patient engagement and outreach.
• Knowledge of community resources and services available to enhance integrated care for patients.

Compensation and Benefits

• Competitive salary in the range of USD 150,000 to 180,000 annually , dependent on experience and qualifications.
