Work Environment

As part of the Home Care division, the Nurse Practitioner will operate onsite, primarily in patients’ homes. This setting requires adaptability, strong clinical skills, and the ability to connect with patients in their personal environments. Working within a team-oriented structure, you will collaborate with other healthcare professionals, leveraging collective expertise to deliver optimal patient care. Key Responsibilities

• Conduct comprehensive initial patient histories and perform detailed physical examinations to assess health status.
• Establish, document, and communicate individualized plans of care, ensuring all patient needs are addressed throughout treatment.
• Perform daily patient rounds, following up on existing patients and evaluating their progress.
• Order medications, laboratory tests, and necessary diagnostics to facilitate accurate treatment planning.
• Interpret laboratory results and diagnostic tests to make informed clinical decisions.
• Work in collaboration with attending physicians, residents, nursing staff, and other healthcare providers to deliver coordinated, high-quality care.
• Complete thorough discharge summaries, including prescriptions and referrals, and provide patients with clear post-discharge care instructions.
• Engage in ongoing performance improvement initiatives, contributing to the advancement of healthcare practices within the organization.
• Participate in research and educational pursuits to elevate the standard of care and enrich personal professional development.
• Undertake additional duties as required to support the overall success of the home care team.

Patient Population / Client Focus

This role specifically caters to a diverse patient population, including older adults with chronic conditions, patients recovering from hospitalization, and individuals requiring complex care management. Your approach will be holistic, addressing both the physical and emotional needs of patients and their families, and providing essential education to empower them in their health journeys.

Qualifications

• A Master’s Degree from an accredited Nurse Practitioner program, with national board certification in your specialty area.
• Active Nurse Practitioner and Registered Professional Nurse licenses in New York State.
• Experience with a collaborative practice agreement, documented protocols filed with the NYS Department of Education, or an attestation of collaborative relationships in accordance with the Nurse Practitioner Modernization Act of 2015.
• Current Basic Life Support (BLS) certification is essential.

Preferred Experience

Prior experience in internal medicine, geriatric care, or home health settings is strongly preferred. Familiarity with electronic health record (EHR) systems and care coordination tools will enhance your efficacy in managing patient care effectively while promoting seamless communication among the healthcare team.

Compensation and Benefits

This full-time Nurse Practitioner position offers a competitive annual salary ranging from USD 150,000 to 160,000, commensurate with experience and qualifications.
