Key Responsibilities

• Collaborate with physicians and a multidisciplinary team to develop and deliver healthcare plans that encompass the full continuum of care.
• Conduct thorough assessments by obtaining detailed medical histories, performing physical exams, and evaluating psychosocial factors relevant to patient health.
• Implement preventive care measures through non-invasive screening processes tailored to specific age groups and health histories.
• Utilize clinical judgment to formulate differential diagnoses and develop treatment plans based on comprehensive assessments.
• Prepare and execute patient educational materials that empower residents to take charge of their health and understand their treatment options.
• Prescribe and manage pharmacological treatments and non-pharmacological interventions effectively, aligning with best practices in Internal Medicine.
• Ensure compliance with established healthcare protocols and continuously monitor patient progress, making adjustments to care plans as necessary.

In this role, you will primarily care for elderly residents living in an assisted living facility. Qualifications

• A Master’s Degree in Nursing with a specialization in Nurse Practitioner.
• An active Registered Nurse (RN) license in the state of New York.
• A minimum of 3 years of clinical experience in Internal Medicine or a related field, demonstrating a strong competency in managing diverse patient needs.
• Proficiency in electronic health records (EHR) systems and familiarity with medical technologies relevant to the role.
• Exceptional communication skills, particularly in patient education, advocacy, and collaborative practice.

Compensation and Benefits

CareMed offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
