This full-time role offers a competitive salary ranging from USD 150,000 to 160,000 annually. Key Responsibilities

• Conducting comprehensive patient assessments to evaluate cardiovascular health, including reviewing medical histories and performing initial physical exams.
• Developing, implementing, and modifying individualized treatment plans based on patient needs and clinical protocols.
• Ordering appropriate diagnostic tests, including EKGs and blood work, and interpreting results to facilitate accurate treatment decisions.
• Operating and maintaining essential medical equipment, ensuring its efficacy for patient evaluation and care.
• Educating patients about lifestyle modifications, medication management, and the importance of regular follow-ups in cardiology.
• Facilitating communication across the care team to ensure comprehensive management of the patient’s journey.
• Providing timely and clear communication of diagnostic results and treatment options to patients and their families, fostering a supportive environment.
• Making referrals to specialists as necessary and coordinating care to optimize patient outcomes.

Patient Population / Client Focus

This role will primarily focus on adult patients experiencing heart-related issues, from routine care to chronic disease management. You will work with a diverse clientele, including elderly patients with chronic conditions, younger adults needing preventative care, and individuals from underserved communities who may face additional health barriers.

Qualifications

• Current Nurse Practitioner certification from a recognized professional body.
• An active nursing license in the state of New York, ensuring compliance with all state regulations.
• A Master’s degree in nursing from an accredited institution, with a specialization in cardiovascular care preferred.

Preferred Experience

• A minimum of 2 years of clinical experience, ideally within cardiology or related fields, with a preference for individuals experienced in community or public health settings.
