Work Environment

This position will be primarily onsite with home care responsibilities, allowing you to deliver personalized treatment within the familiar surroundings of the patient's home. Key Responsibilities

• Conduct in-depth cardiovascular assessments and screenings for patients, analyzing diagnostic tests such as ECGs, echocardiograms, and lab results to inform treatment strategies.
• Diagnose and manage a wide range of cardiac conditions, including but not limited to hypertension, heart failure, and arrhythmias, while implementing evidence-based preventive health measures.
• Design and deliver community heart health education programs in partnership with local organizations to improve cardiovascular awareness and preventive practices.
• Advocate for patients’ healthcare rights, ensuring access to necessary cardiology services while providing culturally sensitive care tailored to individual patient needs.
• Maintain meticulous medical records in accordance with industry regulations, ensuring accurate documentation that supports continuity of care and clinical decision-making.
• Continuously update knowledge on the latest developments in cardiovascular health trends and best practices to ensure our patients receive the best possible care.

Patient Population / Client Focus

Your work will primarily focus on diverse patient groups, including individuals with both acute and chronic cardiac conditions, as well as patients at risk of developing cardiovascular issues. Qualifications

• A Master’s Degree in Nursing from an accredited institution is required.
• Holding an active New York state nursing license is essential, along with relevant credentials demonstrating expertise in Cardiovascular Nursing.
• Candidates should possess a minimum of 3 years of clinical experience, emphasizing a background in cardiology or cardiovascular health.

Preferred Experience

• Experience in community health settings or public health initiatives aimed at improving cardiovascular outcomes is highly desirable.

Compensation and Benefits
