Position Overview

Gotham Enterprises Ltd is actively seeking a skilled and motivated Nurse Practitioner (NP) to join our dynamic outpatient practice within the Center for Advanced Cardiac Care (CACC). This full-time position, located in Merrick, New York, offers the opportunity to contribute to a leading cardiology clinic while providing high-quality patient-centered care. The NP will report directly to the Chief Nurse Practitioner, working closely with our esteemed Medical Director and a collaborative team of healthcare professionals.

Work Environment

This position is based onsite, facilitating a hands-on approach in a personalized concierge medicine environment. Key Responsibilities

The Nurse Practitioner will undertake a broad range of clinical duties including, but not limited to:

• Conducting comprehensive and focused histories and physical examinations for patients.
• Ordering and interpreting diagnostic tests to establish diagnoses and manage both chronic and acute health issues.
• Collaborating with physicians and healthcare colleagues to implement and adjust treatment plans based on patient responses and evolving clinical data.
• Providing patient and caregiver education on disease management, health maintenance, and care strategies in outpatient settings.
• Coordinating care transitions and facilitating hospital admissions, ensuring seamless communication with inpatient teams.
• Documenting all patient interactions accurately in electronic medical records (EMR), ensuring compliance with clinical protocols and reporting standards.
• Participating in multidisciplinary team meetings to integrate care and optimize patient outcomes.
• Engaging in research activities, contributing to the understanding of cardiology practices through evidence-based approaches.
• Maintaining a safe and therapeutic environment for all patients and staff.

Patient Population / Client Focus

The Nurse Practitioner will primarily care for adult patients with cardiovascular conditions, including but not limited to heart failure, pulmonary arterial hypertension, and those requiring post-transplant management. The role demands sensitivity to the complex healthcare needs of these populations, particularly those undergoing advanced medical therapies such as intravenous medication support or mechanical circulatory devices.

Qualifications

To qualify for this position, candidates must possess:

• A Master’s degree in Nursing, with a focus on Nurse Practitioner training specific to adult care.
• A current New York State Registered Professional Nurse (RN) and Nurse Practitioner (NP) license.
• NP board certification.
• Certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), and a valid DEA registration.
• Proficiency with electronic health record systems and familiarity with Microsoft Office Suite, including Word, Excel, and PowerPoint.

Preferred Experience

While a minimum of two years of relevant experience in a clinical setting is required, candidates with a history of working in cardiology or related specialties will be preferred. Experience managing advanced heart failure patients and familiarity with outpatient therapies for cardiovascular issues will set candidates apart. Additional experience with interdisciplinary collaboration is also highly desired.

Compensation and Benefits
