Work Environment

This role is primarily onsite, offering a supportive and collaborative health care setting focused on home care. As a Nurse Practitioner at Eastside Internal Medicine, you will work in close proximity to both colleagues and patients, fostering teamwork and communication. Your schedule will generally include weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also participate in community outreach efforts or health fairs during weekends, contributing to our mission of broader health engagement.

Key Responsibilities

The responsibilities of the Nurse Practitioner will include:

• Conducting thorough health assessments and screenings for individuals and families to gather comprehensive health histories and identify needs.
• Diagnosing, managing, and treating both acute and chronic medical conditions while promoting preventive care and wellness activities.
• Collaborating with community organizations to establish and promote health education initiatives tailored to the specific needs of the population.
• Providing culturally competent care and advocating for patients to ensure their rights and access to necessary healthcare services.
• Coordinating patient referrals and follow-up care to maintain a continuity of care model, ensuring patients receive appropriate support throughout their treatment plan.
• Maintaining accurate medical records in compliance with healthcare regulations and standards.
• Participating in health fairs and outreach events to raise health awareness and deliver vital health screenings to the community.
• Staying informed on community health trends, governmental policies, and evidence-based practices to provide the best possible care.

Qualifications

To qualify for this position, you must have:

• A current certification as a Nurse Practitioner.
• An active nursing license in the state of New York.
• A Master’s Degree in Nursing from an accredited educational institution.

Compensation and Benefits

Eastside Internal Medicine offers a competitive salary range of $120,000 – $220,000 annually, based on experience and qualifications.
